How to configure a Risk Assessment form using the Drill-Down Select block

Use the Manage Templates tool to build structured risk assessments with the Drill-Down Select block.

Access the form template

  1. Log in to BuildPass Admin

  2. Go to Forms

  3. Select Templates

  4. Open the form template you want to edit


Add the Drill-Down Select block

  1. Click Add block

  2. Select Drill-Down Select

  3. Place the block where you want it to appear in the form


Configure the drill-down structure

Define completion rules

  • Enable require complete selections if users must reach the final level (e.g. controls)

  • Disable this if partial selections are acceptable


Create levels and options

For each level:

  • Set a label (e.g. Hazard, Scenario, Control)

  • Choose single-select or multi-select

  • Add options for that level

  • Optionally add descriptions (these appear in the form and PDF)

You can create up to six levels.


Enable custom entries (optional)

You can allow users to add custom items at any level (for example, a custom control not listed).


Add supporting blocks

Common additions for risk assessments include:

  • Evidence uploads

  • Notes or observations

  • Actions

  • Sign-off blocks

These can be added before or after the Drill-Down Select block.


Saving changes

Once saved:

  • Changes apply to new form submissions

  • Existing completed forms are not altered
